Response Form

Please note: This Response Form must be printed out and sent via regular mail, along with a donation, in order to activate daily prayer work in your behalf (see options below). Rent-A-Monk is unable to accommodate any online orders for prayer services.

After you print out the form below, please mark the appropriate box, fill in your (confidential) contact information, and mail it with your check made out to: Heartwings Foundation, P.O. Box 31161, Seattle, WA 98103.

Upon Rent-A-Monk's receipt of your completed form and donation in the mail, your daily prayer service will begin immediately. Rent-A-Monk will also send you a personal acknowledgment, along with your Special Request Focus Cards (see below).

Thank you in advance for your cooperation and respect in requesting Rent-A-Monk services via regular mail only.  

Basic Package: Includes daily prayer work in your behalf, plus two Special Request Cards to keep or return. Also includes additional "amped up" spiritual work to support you. Plus a personal note in the fifth week, with Response Form for optional renewal.

Seven weeks for $35 - $70 ($5 - $10 weekly donation).
Benefactor: Basic Package (above) at $10/week, plus a personal donation to support Rent-A-Monk's free services for those in need.

Death Rate: If you are close to death, Rent-A-Monk would be honored to donate its services to you (see Basic Package above).  

Seven weeks for free (this one-time offer is free even if you don't die "on time").
